How Medicare Agents Make Money
Medicare agents earn $601 per new enrollment plus passive renewal income. Here is exactly how commissions work.
The first question everyone asks before getting into Medicare insurance is: how does the money actually work? First-Year Commission: When you enroll a client in a Medicare Advantage plan, the carrier pays you a commission. In 2026, the CMS-regulated maximum for a Medicare Advantage first-year enrollment is $601. This is set by the federal government (CMS) as the Fair Market Value. Every carrier pays the same maximum rate. A cheaper plan does not mean a smaller commission. Renewal Commission: When your client stays on their plan for a second year, you get paid again — $301 — without doing anything. Without making another call. Without submitting another application. Your client simply renews their plan, and the carrier sends you a check. This is the passive income structure that makes Medicare different from almost every other sales job. The Math on a Book of Business: Say you enroll 200 Medicare Advantage clients in year one. At 85% persistency, you have 170 active policies going into Year 2. Year 2 renewals alone: 170 x $301 = $51,170 — without writing a new application. Meanwhile you are adding another 200 clients in Year 2. By Year 3, your renewal income alone could exceed your first-year production income. The book compounds. Ancillary Cross-Sell: Every Medicare client is also a candidate for supplemental products — hospital indemnity, critical illness, accident coverage, dental/vision/hearing plans. These add another $150-400 per client depending on what they take. A Medicare agent who cross-sells ancillaries on 30% of enrollments adds 20-30% to their total income. Real Annual Income: 2 apps per week = $124,908/year in first-year commissions. 4 apps per week = $249,816/year. 6 apps per week = $374,000+/year. Override Income: When you build a team and recruit agents under you, you earn override commissions on their production without making any of the calls yourself. One manager with 10 agents writing 4 apps/week earns $100,000+ per year in override income on top of their own production. Chargebacks: If a client cancels their plan in the first 12 months, a portion of your commission is taken back. The way to minimize chargebacks: place the right plan for the right client.
